Issuing a copy of medical records or certificates

Issue Info
Medical institutions have the responsibility to maintain the confidentiality of medical records for the protection of patient's personal information. Access to and issuance of copies of medical records require the patient's consent.


Issuance Copy Information

Required documents
  1. A valid identification card or a copy of the card with a photo.
  2. A certificate of family relationship that clearly indicates the relationship with the patient.
         A health insurance card is not accepted as proof of family relationship
         Only certificates issued within three months of the medical record copy issuance date will be accepted.
  3. Handwritten signatures are required on consent forms and delegation of authority letters. Stamps or seals are not accepted
  4. Consent forms must clearly state the scope of the medical record copy issuance, including the date and range of the record.
  5. In the case of a deceased person, a person in a coma, or a minor, a legal representative may act on their behalf

  • If the patient's consent is not possible, a relative can be issued only when both relevant and common documents are brought.
  • Scope of Relatives: Patient's spouse, ascendant/descendant, spouse's direct ascendant (Article 17 Paragraph 1 of the Medical Act)
  • If there are no relatives at all: Issuance to brothers and sisters is possible only when documents proving that there are no relatives are presented.
  • If there is no relative and siblings apply: To prove the relationship with the patient, ① Family relation certificate based on patient standard and ② Family relation certificate (or copy of expulsion) based on parent standard are all provided.
  • If a relative has appointed an agent, it is possible to apply as an agent.
    (① Relevant documents that the patient cannot consent to, ② Relative ID, ③ Documents proving family relations, ④ Consent from relatives, ⑤ Bring the ID of the agent)

(However, to facilitate smooth issuance of documents, please visit the office 30 minutes before the closing time.)

Fee required
※ According to Article 18-1 of the Enforcement Regulations of the Medical Act, various certificates must be kept for 3 years. Certificates that have exceeded the retention period cannot be issued upon request.
